Ph.D. Admissions Checklist

"	Completed Application for Graduate Study available online at http://grad.msu.edu/apply.htm. Your answers to Question #17 should be 2284 for the Ph.D. degree major code and Kinesiology for the major name. (If you do not apply on-line, send completed Application for Graduate Study and application fee to KIN graduate studies secretary and make your check or money order payable to Michigan State University).
"	Two-page written statement indicating your selected area of study and your professional goals. Your response to the personal statement field (Item #22) on the Application for Graduate Study is not sufficient. The Department of Kinesiology requires a separate two-page statement.
"	Writing/research sample (e.g., master's thesis or project, published article, research presentation, paper written for a graduate course)
"	Resume
"	One official transcript from each of the colleges and universities you attended
"	Official Graduate Record Examination (GRE) scores report from the Educational Testing Service
"	Three completed KIN Recommendation for Admission forms. These forms should be sent directly to the KIN graduate studies secretary by the individuals you select as your referees.
"	Graduate assistantship positions: If you are interested in a teaching assistantship position, submit the Application for KIN Graduate Teaching Assistantship: Teaching Competency Form. If you are interested in a research assistantship, contact faculty members in your disciplinary area. If you are interested in an athletic training assistantship, submit the Application for KIN Athletic Training Emphasis.

"	Statement of Financial Proof available online at http://grad.msu.edu/apply.htm - the Statement of Financial Proof is part of the Application for Graduate Study
"	Official Test of English as a Foreign Language (TOEFL) scores. A minimum average score of 580 is required for the paper-based version of the test. A minimum average score of 237 is required for the computer-based version of the test.
